Submit a nomination or request via the forms below. Each form is reviewed by the committee at the next scheduled meeting.

All forms are submitted directly to the Blues Committee via Airtable. You will receive a confirmation email once your submission has been received.

Clubs are welcome to apply for status with the Committee provided they meet specific conditions. Status is awarded at the discretion of the Committee, as voted on by a full meeting of member clubs.
